The Hemodialysis Center of CTKMC-ULP is located on the 4th floor . Gentle Care Hemodialysis can serve up to 10 patients per shift. It’s operation started on May 2019 .
Clinical Requirements:
• Hepatitis Profile within 6months ( Anti HBS, HBSAg, AntiHCV)
• Latest Laboratory Results
• Latest Treatment sheets from previous hemodialysis institution ( 3 consecutive treatments)
• Philhealth consumption
• MDR
• PDD – Philhealth Dialysis Database
• Doctor’s Order
• Swab Test
• Medical Abstract

Monday to Saturday
7:00Am onwards
